- There is a limit of one receipt per customer per visit. Also, the validation code for the reward expires 180 days from the date it was issued.

- Visit the Dunkin Donuts Guest Satisfaction Survey Page.
- Select whether you want to continue the survey in English or Español.
- Enter the 18-digit survey code number that you will find on your receipt.
- You will start the Dunkin Donuts survey off by answering questions such as whether you had to wait in line and what type of food and/or drink products you purchased.
- Afterward, you will start to rate your satisfaction with a number of areas from the friendliness of the staff to the taste of your food and/or beverage.
- You’ll answer additional questions until you near the end where you’ll respond if you’re enrolled in the new DD Perks Loyalty Program. If so, you’ll answer additional questions.
- You’ll fill out personal information like your age and gender, and you’ll be asked whether or not you wish to include your email for future communications.
- Once you’re done, you will receive a validation code. You will need to write it down on a specific location on your receipt.
